Does it shows the browser notification bar instantly as soon as you visit webiste?, or you have to click on the "bell icon" first?

Yes. Very first time it will show icon in corner, which will tell customers to subscribe first, to allow browsers notifications, and if they enable it then he will receive global promotional message that you send from onesignal, to all subscribers.
Any website, which uses push notification, will ask first to allow notifications, that is dependent on customer to allow or block notifications.
